| This is a free overview of the weight loss surgery options provided through the Theda Clark Medical Center Bariatric Program and Midwest Bariatric Solutions. This info session is mandatory for anyone considering Bariatric Surgery. To qualify; your BMI must be 35 or greater in addition to other factors. Meet the surgeons, learn about the program, and how to work through the insurance process. Meetings are held on Monday evenings from 4:30 -7:30 p.m. Registration is required from each person, you may also bring a spouse or friend. There is paperwork that can be downloaded on-line at www.thedacare.org, under the Obesity heading. Please bring this with you the night of class. PLEASE ENTER YOUR DATE OF BIRTH, HEIGHT, WEIGHT, BODY MASS INDEX, INSURANCE CARRIER AND HOW YOU HEARD ABOUT THEDA CLARK'S PROGRAM IN THE COMMENT SECTION. | Bariatric Nutrition: Continuing the Journey |
| Build on the nutritional information you received before surgery in this one-hour class. Learn how to continue advancing portions and food choices for a lifetime and also work on the emotional aspect of food and hunger. Each person must register separately and have had Bariatric surgery prior to this class. If you have had surgery prior to 2004; the cost is $15.00 payable at the door. If you have had surgery in 2004, the cost of the class has been covered prior to surgery. |

| Bariatric Success Circle |
| This is a support group for individuals who have had bariatic surgery or are contemplating surgery. Meetings are the first Monday of every month from 5-6:30 p.m., the third Saturday of every month from 9-11:30 a.m. and there is a general gathering for support and sharing also on the third Monday of every month from 5-7:00 p.m. The cost is $10.00; payable at the door for individuals having surgery prior to 2004. For individuals having surgery in 2004; the fee will be covered prior to surgery. Each person must register separately. |
